PECULIARITIES OF MODERN PSYCHOTHERAPY OF NARCISSICAL PERSONALITY DISORDER

The article considers the problem of features of modern psychotherapy of narcissistic personality disorder. Emphasis is placed on the lack of research on this issue with the importance and relevance of the topic. A review of modern scientific views on narcissistic personality disorder, features of its formation and the specifics of its psychotherapy. The origin of the term narcissism is considered. The concept of “healthy narcissism” is analyzed. The criteria of narcissistic personality disorder according to DSM 5 are considered. Attention is paid to the characteristics of the typology of pathological narcissism: grandiose and vulnerable type. The specifics and importance of diagnosing narcissistic personality disorder are analyzed. Emphasis is placed on clinical interview as the most effective method for psychotherapeutic practice. The degrees of severity of narcissistic disorder are considered: mild form – a person has some specific problems and is somewhat maladapted; moderate – there are more problem areas and there are pronounced symptoms of narcissistic depression; severe form – constant conflicts, aggressive impulses and self-aggression, suicide attempts. It is emphasized that in the process of psychotherapy of narcissistic disorder it is important to pay attention and work with concomitant disorders. It was found that the more aggressive the client, the more severe the narcissistic personality disorder. It is established that psychotherapeutic methods of treatment of narcissistic disorder are developed within the framework of two main directions: psychoanalytic / psychodynamic and cognitive-behavioral. At the same time, the support of psychotherapists for an integrated approach to the treatment of this personality disorder is growing, with the recognition that different methods can be used synergistically and in stages. For the most part, psychotherapy is individual, but group – can be effective in overcoming the difficulties associated with shame, addiction, self-sufficiency, envy, and it is important to take into account the specifics of which group work may be disrupted.
